Reablement Service

As reported by the Clacton Gazette and following the announcement to close the Durban Rehabilitation Centre at Clacton Hospital, Corner House Care Home are excited to announce, in conjunction with Anglian Community Enterprise (ACE) an alternative Re-ablement facility for all residents of Clacton and the surrounding areas.
The Corner House team have undergone detailed training by ACE and supported by their staff will be providing the re-ablement services in the future. Our aim is solely focused on maximising independence and assisting individuals to return to their own home as soon as possible. The key benefits being: –

• Improved health and well-being
• Independence maintained or regained
• Improved social care

Working with Occupational Therapists our staff will endeavour to ensure that individuals will enjoy the support they require to regain the skills required to return to their own home and be self-supportive after a period of hospitalisation. This will be enabled by an assessment of current abilities and supporting and encouragement to regain the independence necessary. All taking place within the comfortable and homely environment of Corner House where our residents are at the forefront of everything we do, from offering delicious meals & choices, to loving care, in building friendships & understanding individual needs.

Referral to the scheme may be by discharge teams at hospitals or through community referrals. For further information please do not hesitate to contact any of the team at Corner House Care Home who will be pleased to discuss the facilities
in more detail. There will be an extra benefit whereby, our Home Care Service will then be available for continuity of care within the service users own homes.
